Participant Waiver, Release of Liability, Assumption of Risk, and Indemnity Agreement

I. Assumption of Risk

I understand that participation in gymnastics, cheerleading, and athletic activities involves inherent risks, including but not limited to: sprains, strains, broken bones, concussions, paralysis, or even death. I acknowledge that these activities require physical exertion and carry a risk of injury.

I understand that all precautions will be taken for the safety of participants, but I voluntarily accept and assume full responsibility for the risk of injury or accident.

II. Waiver and Release of Liability

In consideration of being allowed to participate in programs, classes, camps, competitions, or any related activities hosted by Island Elite Gymnastics, I, for myself and/or on behalf of my child or legal ward, hereby release, waive, discharge, and covenant not to sue Island Elite Gymnastics, its owners, officers, staff, agents, volunteers, insurers, landlords, and affiliated entities from any and all claims, demands, actions, or causes of action related to personal injury, property damage, illness, or death sustained during or resulting from participation in any activity at or through the facility.

This release applies to both ordinary and gross negligence to the fullest extent allowed by law.

III. Medical Authorization

In the event of injury or medical emergency, I authorize Island Elite Gymnastics and its staff to administer or secure necessary medical treatment for the participant. I understand that every effort will be made to contact me, or the emergency contact listed, prior to treatment. I accept financial responsibility for any medical treatment and associated costs incurred.

IV. Code of Conduct and Facility Rules

I agree to follow all posted rules, staff instructions, and safety protocols. I understand that failure to comply may result in suspension or expulsion from the program without refund. I confirm that:

The participant is in good health and physically able to participate.
I will notify the staff of any existing health concerns or limitations.
I will ensure the participant arrives on time, with proper attire, and behaves respectfully toward coaches and peers.
V. Photo and Media Release

I grant permission for Island Elite Gymnastics to use photos, video, or audio recordings taken during participation for promotional purposes, including website, social media, and printed materials.

VI. Legal JurisdictionThis waiver shall be governed by the laws of the Commonwealth of The Bahamas. If any portion is found to be invalid, the remainder shall remain in full force and effect.VII. Acknowledgment and Signature

I certify that I have read this document in its entirety, fully understand its terms, and voluntarily agree to its contents. I understand that by signing this agreement, I am giving up legal rights and remedies on behalf of myself and/or my child.
